The file is present and owned by root with read & execute permissions to
all.
I tried the system trace program but there was no entry in the output
for this file. The error occurs when I make a request from a browser not
when I start apache. Would this be why it doesn't appear in the trace?
Any more ideas as I'm stumped?

Graham, does that path exist? What the perms are? Notice that under
Apache
you are most likely running under a different username so this can be a
problem.
Another thing could be an open file handles limit:
http://perl.apache.org/docs/2.0/user/troubleshooting/troubleshooting.htm
l#Can_t_locate_F_TestFilter_in_str_consume_pm__in_C__INC____
in any case run this under strace(1) and you will see what the real
problem is (see the URL above)
