Hi, here are some questions / things to try.
 1.  Do you have multiple installations of perl?

 2. Post the full output perl -V output to the list.

 3. post the output of

find /usr/lib/perl5/5.8.0 -type f -name "AuthenNTLM*" to the list.

If you have a startup.pl file post that too.

4. Above the Directory  "/opt/Cordys/Web" line
    try adding
    PerlModule Apache::AuthenNTLM
