In fact your correct it was during morning smoko.  The blonde boy bending
the sticks came back and forgot how to move the stick in the bending machine
so he just rolled it along till he got to the other side.  His re-training
supervisor, who should have been there because the blonde had more than a 10
minute period work break, kept him making the curved ones to hide his
absence.

The supervisor reasoned that there had been so many other changes to the 80"
that no-one would notice.  The people making the tilts assumed that it had
merely been a pattern change that no one informed them about.

What is more interesting is that it even made it into the parts books for
the same reason.
