At a quick glance this looks fine. Try displaying your form_ok variable right about the if block and see if it is actually a true value.
Use something like:
Value: '[% form_ok %]'<br>
If form_ok is '0' or '' then you know it's false, and your problem lies above.
You didn't give us the rest of the source of what's going on above that form_ok part so I couldn't really tell ya much more than that.
