many-to-many relationship and sorting

How does the mapping method work internally?
I thought it retrieves all id's of the foreign class (table) with a singl=
e sql statement...then only an ORDER BY sql clause would be needed...

Because there's no way that it can order them without fetching them all
and then sorting them.
