If you're looking for an open source solution to this problem (which I 
understand has recently garnered its own supercool buzzword: Ajax), you might 
want to look at OpenThought ( http://www.openthought.net/ ).  We use it here 
to do all the things you mention and it's pretty easy (by which I mean most of 
the hard work is done for you).
