Thank you.
I have finally installed Apache-Template under Windows using ppm.
I have discovered that I had a TheoryX repository set in ppm, but it was not
pointing to the right url.

I do have nmake installed and it is in the PATH, but perl makefile.pl gave
me an error before nmake-ing...

Yes, it does.

Try, from a DOS prompt,

  C:\> ppm install http://theoryx5.uwinnipeg.ca/ppms/Apache-Template.ppd
